We provide insurance filing services; we accept Medicare, Medicaid, C.H.I.P., Humana, Blue Cross Blue Shield and most other types of insurance. All payments received from your insurance company will be applied to your account. Payment is due at the time services are rendered, however, if special arrangements for payments are needed, speak with one of our financial planners. They will be happy to assist you.
In case of financial hardship, financial assistance is available through the Southwest Allergy & Asthma foundation and a sliding scale methodology is used. To qualify for financial assistance, please bring one of the following types of income verification forms: (1) 1040 tax return, (2) W2 forms, (3) Payroll stubs (last 3 months) or (4) Certified letter from an accountant or attorney for odd jobs done on an individual basis (this letter must be notarized). This program is designed to assist those individuals who are truly in need, who are not eligible for private or public insurance and who cannot afford treatment.
